DLL stands for Dynamic Link Libraries, a Windows standard for storing shared software libraries. (A library is a piece of code implementing common functionality that many programs may wish to share, for example to connect to the internet, so that not every program has to write the same code over and over again.)
Problems would arise from several causes, including versioning issues (a program installs an older version of a library, overwriting the newer version that is already installed, and breaks another program that needs the newer version) and lack of standards for where to put the library files (the library is there, but the program can't find it).
DLL related problems were most frequent in older, 16-bit Windows, and were common and troublesome enough to be termed "DLL hell".
