The OSHA 300 form is a summary log in which the number of recordable injuries and illnesses (and the lost days associated with them) are summarized for each workplace. The summary must be certified by a company executive and posted from 1 Feb through 30 April in each workplace at a location were it will be seen by entering employees.

The OSHA 300 Log is the "Log of work-related injuries and illnesses". This is the form that an employer would use to record all the employee injuries/illnesses as they occurred throughout a year. The nature of the injury/illness is also recorded.

There is also an OSHA 300A and an OSHA 301 form.

The 300A is a summary of the OSHA 300 form where categories are compiled. For example, the employer would report how many days away from work cases there were for a given year and how many hours were worked by all employees, etc.

The OSHA 301 form is filled out every time there is an employee accident. This is an incident report form that is completed after an accident or injury to an employee. The details of the injury, name of employee, and other relevant information is reported.
