DNA is a template for making RNA. I hope this answers your question.

What is a surgical template?

A surgical template is a clear tray that is used in the production of dentures. The tray is designed to receive an imprint of the dentures.

What is directly formed from this template of DNAserves as the template to initiate protein synthesis?

The intermediate molecule formed between DNA and protein is mRNA (messenger RNA). The process in which the DNA sequence is copied to an RNA sequence is called transcription. The process in which the mRNA template is read to produce protein is called translation (protein synthesis)

What are the DNA Translation enzymes?

To my knowledge there is no such thing as a DNA translation enzyme. DNA is not translated; it is transcribed. During transcription, RNA polymerases create mRNA molecules by reading off from the sequence of the DNA template strand. Then the mRNA molecule is translated by ribosomes that convert that use each successive codon of the mRNA sequence to code for a particular amino acid. This last process - the use of sequence information in mRNA to direct the synthesis of a polypeptide chain - that is translation and it does not involve DNA.

What word describes the synthesis of RNA using a DNA template?

This is called transcription.

If the base sequence of template strand is GCCATTAC what would the base sequence of the mRNA?

The mRNA will have codons AUG-CCA-GUA-GGC-CAC

How does DNA template dictate the sequence of the RNA produced?

During transcription the DNA double helix is separated into two individual strands. Each strand may serve as a template for RNA polymerase, which travels along the DNA structure in a 3' to 5' direction. As it progresses down the strand, RNA polymerase synthesizes a pre-messenger RNA strand that is complementary to the sequence on the DNA template. For example if the DNA sequence on the template was 5' ATACA 3', then the pre mRNA sequence synthesized would be 3' UAUGU 5'. (Remember, RNA synthesis utilizes the nucleotide uracil instead of thyamine).
