A free warranty or none at all. The companies that typically sell these policies on older vehicles are often rip off artists with contracts carefully designed to exclude some of the most often needed repairs. They may call an expensive head gasket repair routine maintenance or a consumable part. Or they may not have an agreement with any shops in your area. Some shops refuse to perform repairs if they will be paid by these extended warranty companies because they have a history of nonpayment, slow payment or underpayment. Even if they do agree to perform the work, the garage was the right to keep your car and charge you storage fees every day until their bill is paid in full by somebody(you). They do not pay storage fees. Check the contract carefully if you sign up. And be aware that several of these companies have gone out of business recently blaming the economy leaving customers high and dry while new companies spring up headed by the same faces. Once your car reaches 'maturity' be prepared to fix it yourself, pay to fix more serious problems more frequently than before, or sell or trade it.
