Best advice is every 3000 miles or every 3 months. whichever comes first. Take care of your Volvo and it will take care of you.

The manual on the 2004.5 5cyl s40 Volvo says every 7500 miles
The weight you should use is listed in the owner's manual and on the oil cap. Use exactly that weight no matter that it has 130,000 miles. Also use a quality filter such as Purolator or Wix.
The oil to turbo is from the crank case. period no special reservoir. If you want to change the oil for your turbo simply drain the motor oil. I change my oil & filter on my '93 740 hotted up 3K miles even though Volvo says 5K miles No problems
Change the oil. It comes on every 10,000 miles.

I would just stick with the same oil you've been using.
it could be a number of things ranging from low oil, worn valve seats, worn or broken springs.
it depends on the car and driver. i drive my Volvo hard, and its turbo charged so i replace the oil every 3000 miles with full synthetic, if its a turbo i would replace it with only full synthetic and at most every 5000 miles. if it is a regular one, any kind of oil will do and change it between 5000 miles and 7500 miles only if you drive very lightly on the car.
