To be awarded it, you must hold the MOS either of 11A, 11B or 11C - while, technically, Cav scouts attached to Infantry units could be exempted and awarded the CIB, internal politics always prevented this from happening, and they'd be awarded the CAB instead. If you reclassify to another MOS after earning a CIB as an 11-series, you may continue wearing it.
Anyone who served as an infantryman(11B), ranger or as a green beret and was involved in a combat action.
I.E. Your vehicle being struck by an IED, receiving direct fire(not indirect fire a.k.a mortars/artillery) and engaging the enemy with direct fire.
Basically you must be involved in active ground combat while in one of the units I stated above. Each war is different so the conditions to receive the award change.
There are four different versions of the badge. The first is the plain combat infantryman badge(CIB) design. Each time you receive the award up until the fourth time a star is added. The final design shows the badge with three stars on top indicating you have received the award the maximum number of times(four).
Remember though to receive the award you must be in an infantry, ranger or green beret unit and tasked as an infantryman of some sort. Just because your in an infantry battalion does not mean you are qualified to receive a CIB.
