The release bearing on the clutch is at fault. Throwout Bearing, replace the Throwout bearing and check the Fork for Damage.

check your fluid level for the clutch master cylinder or the throwout bearing, the clutch release bearing could do this too.
I'm not a mechanic / technician but I believe that would be a throwout / release bearing problem on your clutch
You remove the transmission from the vehicle. The throw out bearing is the bearing around the shaft that goes through the flywheel when the transmission is installed. The bearing clips in easily and is held in place because it goes around the shaft, and the clutch release arm can only move in and out, so the bearing will never be displaced during typical use. Removing the transmission is a big job, and you might as well replace the clutch while you're in there, unless it's already new.
The throwout bearing in your clutch is the likely culprit here. Your clutch may be shot in addition, but you are probably looking at a costly fix here.

Clutch Clutch problem, internal trany problem I have had this same problem. It has also happened to a few friends. 1. Throwout/Release Bearing failure 2. Internal Cluster Rod malfunction 3. Lack of proper lubrication
If there were no operating clearance the clutch release bearing would constantly be in contact with the spring diaphragm causing wear on both the diaphragm and the bearing itself, also the bearing pushing on the diaphragm could cause the clutch friction plate to not be fully engaged and slip.
