No. You do not have to have it to upgrade to CS5. CS5 can either be an upgrade if you already own an older version but it is also a fully functional program on its own. if you do not have any versions of Photoshop you would need to buy and install CS5. Then later on if another version comes out you then can upgrade to that version for cheaper.
To move from Photoshop 7 to CS4 will require a purchased full copy of Photoshop CS4 as there is no upgrade available for versions of Photoshop older than 8 - the first CS version.

I'm not 100% clear on what you mean by "upgrade it later". There's no difference (other than price) between the Adobe's "educational license" software and the retail versions. You can't get Adobe's upgrade plan for software you buy at the special educational price, but the software itself is supposed to be identical to the full version... you don't need to upgrade it, unless a new version comes out, and even then the educational price is generally pretty similar to the cost to upgrade from the previous version.
