A platoon sergeant is someone who is responsible for the care and well-being of a platoon of soldiers. It is a description of a job. A Sergeant First Class is a pay grade (rank). It is also called E-7, and abbreviated SFC. Usually (but not always) the job of platoon sergeant is given to a soldier in the pay grade of Sergeant First Class, due to their experience and skill. It is possible to have a well-performing soldier of the previous rank (Staff Sergeant) doing the job of platoon sergeant, and it is also possible that a Sergeant First Class could be doing something other than leading a platoon (staff duty, for example).

There are several ranks of sergeants. Buck sergeants (three chevrons) and staff sergeants (three chevrons with one rocker) led squads, usually of about twelve men. A platoon had four squads. There was a platoon sergeant (three chevrons, two rockers), the top enlisted man in a platoon. The platoon commander was supposed to be an officer, a second lieutenant or a first lieutenant. Platoon leaders got killed a lot. Platoon leader was the single most lethal job of the war. When the lieutenant went down the platoon sergeant took over, until a new lieutenant was assigned. So, squad leaders reported to either the platoon sergeant or the platoon commander, and the platoon sergeant reported to the platoon commander, unless he had become the platoon commander. If the platoon sergeant was leading the platoon he met with the captain or the first lieutenant commanding the company, with the other platoon commanders, when officers were called to company HQ to discuss plans or receive orders.
A platoon sergeant in the army is typically a staff sergeant (E-6). However, it is worth noting that in some cases, a platoon sergeant could also be a sergeant first class (E-7). The specific rank can vary based on the unit and branch of the army.

The smallest element in the US Army structure is a fire team. A fire team is made up of 4 soldiers and is typically lead by a Sergeant (E-5) or a Corporal (E-4). A fire team is part of a Squad. A squad is made up of 9 soldiers and is split into 2 fire teams of 4 soldiers and is typically lead by a Staff Sergeant (E-6). 4 Squads make up a Platoon element. A Platoon element is typically made up of 4 squads and is lead by a Platoon Sergeant, usually a Sergeant First Class (E-7), and a Platoon Leader, usually a 2nd Lieutenant (O-1).
Think about it for a minute: Who writes the NCOER for E-5 Sergeant "Bob" in third platoon? Obviously SSgt. "Bill" the platoon Section Chief or SFC "Tom" the Platoon Sergeant. Who writes the NCOER for the unit First Sergeant? The unit commander or XO, or a Sergeant Major at Battalion or Brigade (more approriately the unit CO or XO). The SMOA has a chain of command just as every other soldier in the US Army. He answers to someone.
