If your making a cake, it could result in it being too sticky (too much milk) and not cooking properly, or not coming together (not enough milk) and being wayy too dry.
It depends very much on the recipe in which the substitution is made. Buttermilk has a different acidity than whole milk, as well as less fat. Additional baking soda will need to be added and perhaps a bit of oil or butter, if buttermilk is substituted for whole milk.

For a good the rises well and does not sink. Baking powder is a 1:3 mixture of baking soda and cream of tartar. Some recipes may need additional baking soda for rising effect because adding additional baking powder would cause the good to taste of too much cream of tartar (it has quite an acidic taste). Not that baking soda is flavour-free, but it is stronger so you need less of it.
