The rough opening of an interior door is two inches wider than the width of the door and two inches higher than its height. Thus your rough opening width is 38". A typical interior door has a height of 80" which would require a rough opening height of 82" but check your door to ensure that is theheight.

Make your rough opening 2 1/2 inches larger on the width and height.
It is not a problem at all. But you will have a 2" gap between the back of the tank and the finished wall.
You cut the drain line and add a 2" section.
depends on the bid. most plumbers include all PVC drain pipe, water supply pipe, fittings, glues and primers, labor to do the job. also if ditches are to be dug this should be stated on the bid. a rough is only the pluming to and from the house that is usually to be covered by cement. the next step is called a top out. it is done after the framing is finished but be for rhe wallws and floors are covered. then comes setting fixtures, this is usually the finial step. Make sure the plumber or caontractor spells this out for you. Do not let them use general terms. If they say there going to do something then it shuold be written in.
