The lung disease emphysema results in the breakdown of elastin in the connective tissues in the lungs. The alveolar walls get broken down so that instead of having lots of little ones to maximise surface area and allow adequate gas exchange, we now have fewer, larger ones that offer a much lower surface area. This means less oxygen gets across and is why you see sufferers out of breath or on oxygen. Air becomes trapped and therefore useless.
