If you mean a medical device- the automatic injector pen (used by people in emergencies for severe Allergies), it is used at a right angle to a fleshy part of your body. READ THE INSTRUCTIONS THAT COME WITH THE EPI PEN.

The best angle is as parallel to the air stream as possible, but due to manifold designs/space and injector design, an angle is need. As per EFI university instructor, " the angle should be less than 45 degrees." The angle has the most affect during idle when the air flow is low.
