Added to answer: I disagree with #1, that you "need" Dreamweaver and Flash. You certainly need Photoshop (or something comparable and really, there isn't a good alternative) but you don't need Dreamweaver and you don't need Flash unless you intend to design Flash sites.
I use Photoshop to "sketch" a design from various elements, then slice up the graphics and hand code in Smultron, a free text editor for the Mac. Which brings us to hardware - get a Mac if you're going to be doing graphic design. I have a beautiful iMac that I find inspiring every time I turn it on! Seriously though, you can design on any machine that will let you use Photoshop. (but Mac rocks)

Frontpage isn't really used anymore for web design and Photoshop is minimally used for doing specific tasks while building a website. Visual Studio (Microsoft) and Dreamweaver (Adobe) are probably the most used.
