It depends on the hypertension medication. If you want a pain killer that does not increase blood pressure, then paracetamol is a good choice. The worst choice would be anti-inflammatory drugs such as ibuprofen and diclofenac. These medications can increase the blood pressure by themselves. In addition, they tend to interact with hypertension medications. For example, these medications increase the risk of renal failure with ACE-inhibitors and sartans, and may also increase the risk of hyperkalaemia with these medications too. Opioid analgesics do not increase blood pressure, however they may interact with anti-hypertensives that are drowsy, such as methyldopa.

Your best bet is to check with a pharmacist or doctor before using anything because it can get very complex.

I currently take high blood pressure medication, but have the flu with body aches. What type of pain reliever may I take? It appears ibuprofren causes the lower glads of my neck (just below the thryoid) to enlarge; looks like a linebacker. I've recently wondered if I have an a reaction to ibuprofren. So what other medication / OTC pain medicine could I take?

Acetaminophen or an opioid pain medication. NSAIDS (ibuprofen, naproxen, etc) may cause increases blood pressure.
