A transgender person is someone who's gender identity does not match their birth sex. For example, someone might be born male, but they feel inside that they are female. They can transition medically and socially. Socially transitioning means that they might get a different haircut, change their name, ask to be referred to as different pronouns, use different bathrooms, etc. Medically transitioning involves changing their body to match their gender identity, for example, a transgender man (female to male) might get top surgery to remove his breasts. There is no requirement for any transgender person, someone might want almost every surgery, where someone else might only transition socially. Whatever is right for them.
Transsexual means the same thing, but it is an outdated term that is barely used anymore.
