First course of action would be to check for a coolant leak. When it's sitting in the driveway is there a puddle under it? If it's burning coolant there will be white soot in your tailpipe. Failing that I would check(replace for ~$15) the thermostat, it's the temperature sensitive valve that regulates coolant flow, it may be stuck closed.

jump power from the battery to the fan directly to see if the fan operates. if not, replace fan. interchange another relay with the fan relay to check the relay. there are usually the exact same relay located in the engine bay.
