Those close to Padre Pio attest that he began to manifest several spiritual gifts including the gifts of healing, bilocation, levitation, prophecy, miracles, extraordinary abstinence from both sleep and nourishment (one account states that Padre Agostino recorded one instance in which Padre Pio was able to subsist for at least 20 days at Verafeno on only the Eucharist without any other nourishment), the ability to read hearts, the gift of tongues, the gift of conversions, and the fragrance from his wounds.

Numerous Church and medical personnel examined his wounds but could not find a natural explanation. They did state that carbolic acid could not have cause the bleeding as the chemical would have destroyed the tissue and caused it to rot. Finally, tired of all the publicity, the superiors of Padre Pio forbade him from showing the wounds to anyone, including Father Gemelli. Just before Padre Pio died the wounds disappeared, leaving no trace.

One notable miracle was witnessed by then Bishop Wojtyla (later Pope John Paul II). He maintained a close friendship with a Polish couple who had 4 young children. Bishop Wojtyla learned that the mother was suffering from stomach cancer and it was apparently inoperable. The Bishop sent a hand delivered note to Pio asking for his prayers. A few days later the doctors agreed to operate to see if anything at all could be done. However, the surgery was canceled when the morning of the scheduled surgery they could find no trace of the tumor. It had disappeared overnight.

Padro Pio is credited with exhibiting stigmata, painful marks on his hands and feet, seemingly consistent with the nails driven through Jesus' hands and feet. From what we know of Roman practice, a long nail would have been driven through Jesus' heels rather than in the customary place for stigmata, but that is a mere detail. According to Agostino Gemelli, Padre Pio was "an ignorant and self-mutilating psychopath who exploited people's credulity." There was a suggestion, dismissed by the Vatican, that the stigmata were produced by carbolic acid.

Padre Pio was enormously popular during his lifetime, and after his death people came forward claiming miraculous cures from his intercession. Pope John Paul II canonised him in June 2002. His body was exhumed; his hands found to have no evidence of stigmata.
