Yes I have. With supervisors it often been because there is an "We've never thought of that." or a "We've tried that before." or a "You're new here, what do you know." approach to things. With instructors it's usually a matter of "Don't give me any new ideas. I've been teaching this course for 20 years." or "I'm the teacher, you're the student, remember."

hi there, normally with restaurants the amount of waiters is too small for them to have supervisors, so if you experience a problem or would like to praise the waiting staff, then just ask the waiter, or one which is not waiting on you, to speak to the duty manager. josh i hope this is what you meant.
Yes, supervisors do have their own email address.(See Related question regarding the format of the email address.) Supervisors are issued with an official answers.com email address. A supervisors email address normally consists of the supervisors username followed by @answers.com so if you were emailing me it would be RHYFOR@answers.com as an example.
