Ok, you have 2 options here. Either the lesser demons on Karamja, or the flesh crawlers in the Stronghold of Security.
In my opinion, defence for higher levels is not that hard, but takes time. Attack and strength are good to raise. I recommend training all of them.
Only slightly, you will also need more strength and attack than defence to lay harder hits.
You can select an attack type that gives you only defense experience. Note that this will give you no attack or strength experience.
Probably training on Fir Giants at baxtorian falls.

When you attack, make sure your attack style is 'block'. This means when you attack something, you gain experience in defence only.
