You should hang a mirror at least 6 feet high from the floor when hanging it behind a dining room table. You don't want to hang it lower than that because it will reflect the table.
Unfortunately The Bombay Company went out of business. I used to work there and have the Montebello Mirror. I moved and a few of the panels broke so I am no longer able to hang it. Its good to hand pictures at an average eye level. Pictures should not be hung higher than a door frame.
That's a matter of personal preference and the style of the lighting. You CAN get bathroom lights that are attached to the sides of the mirror, but generally they go to the top edge.

It is not good t hang mirror on back of door according to Feng-Shui. But a sturdy mounting and strong support is necessary and preferable mirror should be small so that the weight will be less.
Mount the box one to two inches above where the top of the mirror will be.
