To date, the only declared"hot" war (actual shooting) where a nuclear-powered submarine was involved in an attack on another Naval vessel is the Falklands War in 1982. Britain's HMS Conqueror still remains the only nuclear attack submarine to attack and successfully sink a manned target, the ARA General Belgrano (Argentina).

For the Cold War, USS Nautilus (SSN-571), was the first nuclear attack submarine. Others of course were used during all other conflicts (even though they weren't seen, they were there).

The First Gulf War saw nuclear attack submarines shooting Tomahawk (TLAM) land-attack cruise missiles at Iraq, the first time in history submarines shot cruise missiles in combat.
