Italy was the heart of the Roman Empire. The Roman Empire was not Italy's empire. As the name makes clear, it was the empire of Rome. Since Rome is in Italy , Rome and Italy were the heart of this empire. The Roman Empire was not the heart of anything. it was its own political unit
