The long and slow development of government. However, government was developing a long time before you might suspect it. In England at least, the exchequer (a money counting system still in use in some form today) was introduced in 1110, chancellors (which also still exist) were well established by the reign of Richard I and Henry II took huge steps in creating set-down methods of dealing with different problems, creating much more continuity in both government and law. Basically, effective government depended on the effectiveness of those involved in it (look at the reign of King Stephen in England), just at it does today, and the only solution to an ineffective aspect of government was the development of a more effective method. (However, if this is a school question, the answer you may be looking for is the feudal system, whereby everyone was accountable to his direct superior).
