Respiratory flus usually cause the most deaths since the disease is airborn and easily passed from person to person. Plus, flu can easily turn into pneumonia and most deaths occur from flu-related pneumonia rather than "the flu" itself.
In 1918, most industrialized cities also had considerable air pollution which impairs the respiratory system. Areas with coal mines already had many older miners suffering from what we now call Black Lung. Many other people had chronic Asthma and emphysema but without the respiratory medications we use today. Anyone with a respiratory condition was much more suseptible to respiratory diseases and resulting death from the simple flu.
Even today, flu claims thousands of lives in the USA every year and many more thousands worldwide, and still, for largely the same reasons as in previous decades. The youngest, the oldest, and those with chronic respiratory problems are still at the highest risk of flu-related deaths.
