The loose wire connection, or it may have stalled.

A front or rear hub bearing assembly going bad could sound like that.
The Sport button on the shifter for the 1991 Honda Accord is to change the motor so that it will have a little more power. The RPMs should go up by about 500 RPMs.
Change the plugs ans wires. If it is still there have a wet and dry compression test done.

A fuel pump that is not functioning properly will cause the RPMs to fluctuate. The fuel pump may be going bad, or it may not be grounded properly.

Is it stalling? If not, dont worry about it, your fine. My 90 accord does that everyonce in a while but continues to run.
