There are three possibilties I know of: 1) poor water supply to house, need filtration system. 2) too much clothes in machine. 3) need new washer, if pieces are rusting the rust floats or an oil leak inside the washer will leave a line on the clothes at the top of the machine.

Dye spreads through the clothes in a washing machine when it starts rinsing. Whilst it is rinsing, the water will pick up the dye and then start it's usual spin, this will cause the dye to go all about the washing machine and the clothes pick it up ( The same principals of getting a stain on your t-shirt ), only when a dye is used the chances are it won't come out in a wash.
Cocoa powder will not stain clothes permanently. Shake off as much of the powder as you can. Then pretreat the stain and wash it in hot water.

Flair pen will not cause clothes to rip. It is possible that scrubbing the stain too harshly damaged the fabric. If you treat the stain gently, there is no reason that the clothing should rip.

Machines use hot water, which can set some stains, and wash many clothes at once, which can re-deposit stains on another piece of clothing. Hand washing allows the washer to treat each stain depending on the type of stain and the fabric.
Powder can't ruin your washer i dont think. But when you use powder, it doesnt always dissolve, and sometimes you will have white stuff on the clothes were the powder didnt dissolve? I dont know if that helped.
yes it can. We had this happen and the washer was junked as a result. It seems that the washer is built around the "spin bearing". This bearing sits in the middle of the washer shaft and is what the washer spins on. If it starts to wear out, it can leak grease from the bearing and stain clothes. Our repair man told us that for the cost of repair, we would be better off buying a new one.

Obviously the stain comes from an ethanol based material. - Use it sparingly and then wash in a machine with good detergent.
