I would say yes. Your testicles are very important. If you have ANY ongoing pain in your testicles I would get in my car and RUSH to the hospital. It may be serious and prevent you from having children if untreated.

If you mean testicles, and not scrotum, the condition you are describing is most likely cryptorchism. This condition arises when one (or both) of the testicles do not descend from the abdomen within the first year or so after birth. The testicles are located in the scrotum mainly because the temperature there is lower than the body's core temperature. This slightly lower temperature is optimal for the testicles to produce sperm-cells. When the a testicle does not descend into the scrotum like it normally would, the tissue within the testicle is affected by the slightly higher temperature. The tissue will atrophy (waste away) and the result will be lower fertility later on in life. The condition can easily be corrected by a quick corrective surgical action. Because of the possible long-term consequences, you should contact your family doctor.

Why are horses not able to be castrated with elastic rings such as those used on cattle?
Because of the way their testicles are held in relation to the plane of their body. Stallions and boars are similar in that their testicles are more on a horizontal plane in regards to where the epididemus is; the epididemus tubule, the tube which connects the testicle to the rest of the glands that secrete seminal fluid, is horizontal or parallel with the stallion's underside, the same way the epididemus tubule of the boar is parallel with the angle of the rump, making the length plane of the testicle parallel to the body, in stallions and boars (parallel to the belly and rump, respectively). However, in the bull, the epididemus and long plane (length) of the testicle is perpendicular to the belly, creating a kind of "neck" at the base of the scrotum, and allowing the testicles to hang like they do. When the testes hang like that, this is ideal for being able to place an elastic band at the base of the testicles (or on the neck area), because it does not hurt the testes (a VERY sensitive area), but merely cuts off the blood supply of the veins and arteries that run along the epididemus to the testicle. Stallions nor boars have this neck area at the base of their scrotum: it's all attached to the belly, and putting a rubber ring at this area would be both quite painful, and do more harm than good.In stallions, at the base of the testicle on the epididemus tubule there is this sac called (I believe) the seminal sac. If this is not removed along with the testicle, the gelding would still act like a stud stallion, and not like a real gelding. It's nigh impossible to feel for this little sac when trying to band a stallion or a colt, thus it's better that the more surgical process of cutting off the testicles is used instead.

There is no surgery to reduce testicular size, at least in humans. They can be removed and replaced by smaller prosthetic testicles, but a wedge resection or other similar procedure is not done because it would make the testicle nonfunctional.

Theoretically if the sperm came from the donated testicle, it would be genetically related to the person who donated the testicle. But it is a moot point, because testicles cannot be successfully transplanted.
