It is asking for your most current degree, therefore the last one you completed. I just have never heard of it being asked that way. If this is a job application, I would want them to know I have the MBA, and employers would want to know your entire educational background. However, maybe it is not a job application. That information would have helped.
When asked to submit additional information to support your application, you should include information from other employers. You could also supply a copy of your degree.
If you are asked to write a specific purpose statement on a college application, you should discuss your reason for going to college. You should write about what you hope to gain from a college degree.
No more than they asked on the application when they loaned you the money. Name, current address, current pH #. SSN, ect. the usual. What are they asking for that you cant give?? Ins, info??
If you are asked to submit a career overview with a job application you should summarize your current and former positions. An overview will touch on the jobs you have held that are most relevant to the position for which you are applying.

I wouldn't put the reason on the job application. You can save this for the interview and only if asked for the reason why.
No it does not count as a GED, because they are different. However, because you have the associate degree, the GED is a moot issue, because the associates is a higher level of education. If you were asked on - lets say - job application as to your education, you would indicate associates of arts, and indicate the major.
yes
When you are asked what your expectations of the job application are, you should be optimistic. State your goals and what you intend to do given the opportunity to work with that company.
Every application is different. However, most commonly you will be asked for your basic information (name, SS code, date of birth, address, phone number, etc.) as well as previous qualifications.
Your expectations.
