In 1967 the World Health Organization (WHO) started a worldwide campaign to eradicate smallpox. This goal was accomplished in 10 years due in a large part to massive vaccination efforts. The last endemic case of smallpox occurred in Somalia in 1977. On May 8, 1980, the World Health Assembly declared the world free of smallpox.

Smallpox was the first disease ever irradiated by a vaccine. In fact the smallpox vaccine was the first ever invented. In 1796, Edward Jenner noticed that milkmaids (women who milk cows) that had contracted cow pox, tended to be resistant to smallpox. The milkmaids were contract cow pox which resulted in blisters on their hands but was not fatal. However, these women did not contract small pox. It was discovered that by subjecting people to this milder cow pox virus, people would be protected from small pox. In fact, the name vaccine is taken from the latin word vaca which means cow. In the early to mid-1970's an international effort was undertaken to identify cases of the disease and vaccinate people around the world. By the late '70s the disease was declared "eradicated" by the WHO (World Health Organisation).

The last case in the United States was in 1949, but smallpox was last reported in the world in 1977. However the virus known as variola is still kept in laboratories in the U.S. and Russia for further experimental testing
