This would be a matter of state statutes. Each state enacts specific laws pertaining to real and personal property and how ownership is established. Generally all that is needed to have the title changed is the probate order establishing the validity of the will.

Often you or the mortgage company file a certificate of release, referencing the original record of the mortgage. There are other complications if there was a deed of trust or other partial conveyance.

When you pay off your mortgage the lender must send you a discharge or deed of release which you must record in the land records. Once it's recorded your title is free and clear of the mortgage.
