There are no good sites that I know of where you could find simply a template for this, but the easier thing to do is to get yourself a measuring tape used for clothe sizing and take length by width measurements of your arm. Then, it is simply a matter of marking those measurements on paper, and then sketching out a basic shape of the arm. If you want a front panel sleeve, you will simply draw a cigar shape on the paper with a rounded top to act as the top of your shoulder. If you want a complete sleeve that will wrap around, you will still make the same cigar shaped sketch. For that you will simply draw a rounded top down the center the same as before but you will then do a sketch a downward bended line on either side of the rounded top to the edge of the width measurement. When the sheet of paper is bended around your arm, the lines will meet underneath your arm just below the armpit. Then it is just a matter of filling the blank spot with the designs that you want and taking it to the artist.

The adapter sleeve is typically used when the bearing is on a shaft without a shoulder and needs to be placed properly in a pillow block housing for example. This sleeve uses a hydraulic or mechanical device or nut to tighten the bearing on the sleeve and hydraulic oil injected into the end of the sleeve on larger sizes to release bearing when removing. The withdrawal sleeve is used when the bearing is installed against a shoulder on the shaft and pushes the bearing against the shoulder while tightening. The advantage of this is the bearing can be installed with the shaft out of the machine since it is pressing against a shoulder, with the adapter sleeve, it would be more difficult to position the bearing with the shaft out of the machine since it is free to move on the shaft until it is tightened. The choice of which to use will depend on the design of your shaft and how it will be installed.
