That piyut (poem) is rendered during the leader's repetition of the Amidah during the Shacharit service on Yom Kippur.

The refrain in response to each verse is "l'chai olamim".

Different Yom Kippur machzorim may have more or fewer verses, but that's where you'll find it.

Ha'Aderet Ve'Ha'emunah הָאַדֶּרֶת וְהָאֱמוּנָה


This Poem, or Piyut, is in the middle of the Psukei D'zimrah in the Sefardic Shabbat morning service for Sefardic prayerbooks, and in the Yom Kippur service in Ashkenazic tradition. This piyut, which is also sung by many at tishes and on simchat torah.

It is as follows:
