There are lots of places where you can get the 11th Doctors sonic screwdriver but the cheapest one I can recommend is Forbidden Planet who are selling it for £13.99. Other places I know you can get it from are Amazon, Galaxy 4, Ebay but some of those places can be pretty expensive for one as the cheapest on Amazon is £18.99 and then there isn't P&P charges added to it. Hope I can be of help :) x
You can try and search it on eBay or have a look in some libraries. I think there are also some other Doctor Who websites that would sell them, too.
The sonic screwdrivers from the second doctor to the tenth doctor were made by the doctor himself but the eleventh doctor's sonic screwdriver was made by the tardis because the tenth doctor's sonic screwdriver wouldnt work.

No. Patrick Troughton, the second doctor, was the first to use the sonic screwdriver, and even then, it was a part of the TARDIS repair kit. The screwdriver made appearances in other Doctor Who episodes with later doctors, but didn't become the Doctor's tool of choice until the series rebooted in 2005.
