Surprisingly enough, most of the Iraq War occurred within the borders of Iraq, a country in the Middle East.
More specifically, though, the initial fighting to topple Saddam Hussein was primarily concentrated in the southeast of the country in the Shatt Al-Arab and the Shiite Holy Region (Karbala, An-Najaf, and the nearby regions), terminating with the entry to Baghdad. The majority of the fighting against the militant factions (Mujahidin, Fedayeen, Sadr's Militia et al.) occurred in what is colloquially called the "Sunni Triangle" which had vertices at the cities of Baghdad, Fallujah, and Ba'aquba. There was scattered fighting outside of this area in the southern area of the Kurdish Region in Northern Iraq and in the Western Anbar Province.
It's taken place in many different places, but primarily in the provinces on the border between Iraq and Iran such as Khuzestan and Kermanshah in Iran and Al-Basrah and Diyala in Iraq.
Iraq launched a full-scale invasion of Iran on 22 September 1980. The Iraqi Air Force launched surprise airstrikes on ten Iranian airfields with the objective of destroying the Iranian Air Force on the ground. The attack demolished some of Iran's airbase infrastructure, but failed to destroy a significant number of aircraft. The Iraqi Air Force was only able to strike in depth with a few MiG-23s, Tupolev Tu-22, and Sukhoi Su-17 aircraft, largely ineffective due to Iran's sheer size. Three MiG-23s managed to attack Tehran, striking its airport but damaging only a few aircraft. The next day, Iraq launched a ground invasion of Iran along a front measuring 644 kilometers, in three simultaneous thrusts. The purpose of the invasion, according to Saddam, was to blunt the edge of Khomeini's movement and to thwart his attempt to the Export of the Islamic Revolution to Iraq and the Persian Gulf states. Saddam hoped that by annexing the oil-rich province of Khuzestan that he would send such a blow to the prestige of the Islamic Republic, that it would lead to the downfall of the new government, or at very least put an end to Iranian calls for his overthrow.
Iraq. That's why it's called the IRAQ War.
The first one was fought from 1990 to 1991 in Kuwait and Iraq. The second one has been fought since 2003 in Iraq.

There have been a great many major events that have taken place in Iraq. These events include the Iraq war.
Iraq as an independent nation (since 1923) has never successfully taken over Iran. Iraq's incursions into Iranian territory during the first part of the Iran-Iraq War (1980-1982) occupied less than 5% of Iranian territory. However, there have been empires based in Iraq that have conquered Iran such as the Abbassid Caliphate and Ilkhanate in the 800s-1300s.
The Gulf War took place in 1991, not 1989, and it was sparked by the invasion of Kuwait by Iraq.
