Formats for medical reports are pretty prescribed (no pun intended). A good start is to look in the AAMT Book of Style put out by AHDI. Another place is a book called Medical Transcription Guide: Do's and Don'ts. Most people learn proper report formatting through a quality MT program. You can't just "do" MT w/o quality training. All medical facilities have their own formatting styles. The most common formatting for doctor's office notes is SOAP (subjective, objective, assessment and plan). This is what I would use if they don't give you a specific format.

What is Transcription - WPM?

It depends on the context (whether referring to handwritten or typed). Typically transcription is "something written, especially copied from one medium to another, as a typewritten version of dictation". "WPM" stands for "words per minute"...as in how many words per minute can you transcribe. An example of "Transcription-WPM" could be referring to how many words per minute you could transcribing someone's notes from a meeting into legible format, either handwritten or typed. Transcribing doctor's notes, etc. It really varies depending on the field. Good luck!

Have you checked with your local technical college? The college I go to right now offers a medical transcription certificate that is basically free for Georgia residents through the HOPE grant, and many classes are offered online or in a hybrid format each quarter. Maybe your state and local college offer a similar program

What is the difference between medical billing and coding and a medical transcription?

Medical billing and coding is the process by which payment for medical services is arranged with either an insurance company or a managed care agency, while medical transcription is an allied health profession, which deals in the process of transcription, or converting voice-recorded reports as dictated by physicians and/or other healthcare professionals, into text format. Medical coding is the coding of each patient's diagnosis with an ICD-9 code, which is a code number that is recognized world wide. An ICD-9 (International Classification of Diseases, 9th Edition) book contains a diagnosis code for every illness, disease, injury, etc. known to man, and is used world wide. Medical billing is the process of billing the medical companies, using the transcribed medical information and the diagnosis codes, to give the insurance companies all of the pertinent data required to process payment of medical services. Medical transcription is listening the dictation and typing the dictated notes, and Medical Billing is Assigning the codes and billing. If you are good in listening skills and typing, then choose medical transcription. If you are good in medical terminology and analytical skills choose the medical billing. All of them are now being offered worldwide as there are outsourcing companies for them too.

What kind of record format consists of format databases problem lists initial plans and progress notes?

The record format you are referring to is likely the SOAP note format used in medical documentation. SOAP notes include subjective information (S), objective data (O), assessments (A), and plans (P), making it a structured format for healthcare professionals to document patient encounters.

Is medical billing and medical transcipionists the same?

No. Medical billing and coding is the process by which payment for medical services is arranged with either an insurance company or a managed care agency, while medical transcription is an allied health profession, which deals in the process of transcription, or converting voice-recorded reports as dictated by physicians and/or other healthcare professionals, into text format. Medical billers take the coded medical record, and bill the insurance company while medical transcription is every encounter that takes place between a health care provider and a patient is recorded either by voice or electronically. When dictated, the information is then transcribed (typed) by a Medical Transcriptionist and the report is filed in the patient's chart (hard copy or electronic or both) using a computer and a word processor. The job outook for both field is looking nice actually. The Governments occupational outlook handbook suggests the demand for workers in this industry is going to have more demand for the next decade. Medical billing isn't exactly popular among people in the allied healthcare field. And who could blame them? Most people have a natural aversion to numbers. But what not a lot of people don't know is that medical billing presents a lot of opportunities to those who are willing to study it. This is why a number of companies offer medical billing certification training in conjunction with their basic medical training packages.
