After you have defeated Cagnazzo, Fly to Mysidia and go to the Crystal Room in the Hall Of Prayers. (There is a hidden door on the top-left side of the room, Go through it) Talk to the elder, He gives it to you.
You MUST talk with the elder straight after defeating Cagnazzo or you WON'T be able to get it. It's a MISSABLE item.
