I am confused by your question, because the D train (of the B-D-F-M, the orange line) does not stop at 79th Street. It's an express train, and it runs straight from 59th Street-Columbus Circle to 125th Street.

The other thing I'm confused by is what you're actually asking: "where do I transfer?" You transfer from one train to another inside the station. I would think you would understand that already.

The only train that stops at 79th Street is the 1 train (of the 1-2-3, the red line) at West 79th Street and Broadway. And it is the only train that stops at that station. There is no other train to transfer to.

If you are sure that you're supposed to be taking the Dtrain, then: take the D train to 7th Avenue-53rd Street. Transfer there to the Uptown (Queens-bound) E train (of the A-C-E, the blue line). Take the E train 2 stops to 53rd Street-Lexington Avenue. This station is connected to the 51st Street Station on the 6 train (of the 4-5-6, the dark green line). Take the Downtown 6 to 14th Street-Union Square.

The D train stops at 34th Street-Herald Square (at Broadway) in Manhattan. The 34th Street-Herald Square station is serviced by the B-D-F-M (the orange line) and the N-R-Q (the yellow line).
