There is a fuse box under the hood, next to the battery. The fan relay is in there.

Check the underhood fuse box.
Check the thermal relay for the cooling fan.
Often a cooling fan will stay on simply because the AC compressor is on, and the compressor stays on when you are using the AC or defroster. However, if the fan remains on when the key is off, check the relay. I believe the relay is the same type used for the horn so you could briefly "swap" them to see if the horn remains on. If you find that the relay is sticking, you can purchase a new one at most auto parts stores, just don't go far (5 minutes of engine running at maximum) without a cooling fan; you don't want to run the risk of overheating the engine... that would be expensive.
If when you write "fan control module" you really mean "fan relay", it's easy. The fan relay is usually the same as the horn or AC relay. Check the numbers on top, and if they're identical, simply swap the relays to see if the problem follows the relay or if the fan still doesn't work. The relays are in the fuse box that's close to the battery, and they're labeled on the lid of the box. Carefully work the two relays out of their sockets and simply switch them. If it's a relay problem, most auto parts retailers should be able to find an inexpensive replacement for you.
If the relay is good and the fan is getting power and groun, then the fan motor is bad.
