That is an anti-static wrist strap used by electronics technicians to guard against ESD (electro static discharge) when working on sensitive printed circuit boards and other electronics. The strap connects to a work mat which is in turn connected to earth ground.
You take the wrist strap and look at both ends. One end should have a tinier strap attached to it. You take the tiny strap and look at the back end of the ds. It should have two small holes there. Then take the strap and stick it in one hole. then , push the strap, and it should come out the other hole. Take the strap that came out the hole, and make a loop, by splitting them apart. Slip the actual wrist strap through the loop, and pull it through. Bingo!
The wrist strap is used to prevent static charge buildup, which can arc from you to the PC, and potentially cause damage. Because of this, there is no time when you should NOT wear a wrist strap. It may not be needed at all times, but it certainly doesn't cause an issue if you do. I do not always use this; when it is warm and damp, static charge buildup is less of a problem. Also, if you are working in an area away from things that naturally help build up charge on your person (shag carpet, wool,...), it because less of an issue. But there is always some level of risk involved when working inside a computer and not having this.

An anti-static wrist strap is connected to just the earth circuit in the household mains supply - this sends any build up of static down to earth - preventing it getting to the computer. An anti-static mat is usually made of a conductive plastic. Usually, on one corner there is a snap connector. From that snap connector there is a long, (usually GREEN) wire, as well as a coiled wire that goes to a wrist strap. The green wire goes to the round GROUND connector of an AC wall socket. The coiled wire that goes to a wrist strap goes just like that. Wrap the wrist strap around your wrist. The reason for this setup is, through the green wire to the ground connector, you put both the mat and your body at ground potential. It takes the static electricity from your body,via the wrist strap, and from the mat to ground. Be VERY CAREFULL!! YOU MUST KNOW WHERE THE GROUND IS ON AN AC OUTLET TO USE THIS AS I HAVE OUTLINED. If you do not know where the ground pin is on an AC outlet, you should not use it.
