The wrist strap is used to prevent static charge buildup, which can arc from you to the PC, and potentially cause damage. Because of this, there is no time when you should NOT wear a wrist strap. It may not be needed at all times, but it certainly doesn't cause an issue if you do. I do not always use this; when it is warm and damp, static charge buildup is less of a problem. Also, if you are working in an area away from things that naturally help build up charge on your person (shag carpet, wool,...), it because less of an issue. But there is always some level of risk involved when working inside a computer and not having this.

It is a very good practice to wear a properly grounded static strap when working on a PC. Static discharge can be very damaging to computer components. It's a good idea to be certain that the PC case is also grounded while working. I have worked on and built multiple computers and never worn a static strap or worked on a anti-static mat. I do prefer to touch something metal to see if it shocks me just to be safe. I have never damaged anything from this, but I do know that It can happen.
