The wrist strap is used to prevent static charge buildup, which can arc from you to the PC, and potentially cause damage.
Because of this, there is no time when you should NOT wear a wrist strap. It may not be needed at all times, but it certainly doesn't cause an issue if you do.
I do not always use this; when it is warm and damp, static charge buildup is less of a problem. Also, if you are working in an area away from things that naturally help build up charge on your person (shag carpet, wool,...), it because less of an issue. But there is always some level of risk involved when working inside a computer and not having this.

Yes. You should wear one whenever you work inside a computer.
You do not want to wear a wrist strap or be grounded when working on monitors/TVs or power supplies. Even when turned off these devices still hold a charge due to the large capacitors in them.

You should always where a ground strip when working on the internals of a computer. You should always unplug the computer from everything before working on it and ground yourself to the computers metal case. This will equalize the static electricity between you and the mother board and her components.
