Look online for any U.S. Coast Guard "Regional Examination Center." When you find the one closest to you, simply go in and ask for an application for (what today is) a Merchant Mariner Credential (the card has been phased out.)
You need to pass a physical examination (done by your own physician or a physician of your choice), a drug test and a background check. Once this all clears you'll be called back to pick up your credential and swear in as a Merchant Mariner of the United States.
