QWest only has about five plans currently, and they're all based on internet speeds. By all means, the 40 MBPS is the best they offer and it's only $44/month. Even that's a trickle compared to what countries outside the US get. Be aware that ISPs frequently advertise "best-case scenario" speeds that they can't even deliver most of the time.
Qwest does. Verizon has a plan. I think even comcast does.
The cost of qwest dsl per month varies depending on which plan you decide to get and for how long. For example, if you sign up for a 12-month plan, you can receive qwest for as low as $14 per month.

The iPhone has its own Service Plan which has Internet included.
Unlimited internet is a 'free' service with T-Mobile if you have a plan however the plan is not cheap so your internet isn't really free. It is cheaper than most other cell phone companies though.
An upgrade can be in a plan or software update, it allows for better internet connection as well as more minutes, text, and internet usage allowed on the phone. Usually you also get better surface. To do this go or call your service provider.
