The origin of the idea that the "forbidden fruit" was an apple was caused by human translation errors when translating religious scripture from Latin to English.
The Latin word for "evil" is "Mali", whereas the Latin word for apple is "mala" (or "malum" depending on context). Whomever translated the works from Latin to English misinterpreted the Latin word "evil" for the English translation "apple". Therefore "forbidden fruit" is supposed to actually refer to "forbidden evil".
It is believed by those who follow the relevant religions that the "forbidden fruit" is referring to sexual intercourse (or the fluids of sexual intercourse, depending on the exact branch of the religion you ask). Which itself is somewhat silly, considering without sexual intercourse, the human race (and any other creature banished from such activity) would cease to exist. But the swallowing of sperm and of menstal fluid, is still a sin!

all he asked adam not to do was eat the fruit of the tree of the knowledge of good and evil. after eating the knowledge of good and evil fruit adam was banished from the garden along with eve who also ate the fruit.

However, he did eat of this fruit and he did die, both spiritually and then physically.
