Well, it depends on who your favorite anime character is, and if you're a boy or girl. After you choose who you will cosplay as, you should check a costume store like Party City to look for wigs, dresses, etc. (or just check online).

Cosplay is costume. In Manga/Anime festivals, people cosplay as their favourite manga/anime character. They make their own costume or they buy one, already made. Are you going to a anime convention? You don't need to cosplay. It's not a must... unless there is a rule for that particular convention. You could just wear a wig and a cute outfit, and say that the character you cosplay would wear this too. Or just wear something similar. You don't have to look exactly like that character.
That is not really an answerable question. Cosplay stands for "costume play". Anything regarding someone dressing like an anime or video game character would fall under the general umbrella of cosplay. A more significant question to ask would be, when was the term "cosplay" coined. I believe it was coined in Japan around November of 1993.

As far as I know, there isn't. I've heard a rumor of one in St. George, UT, but if there isn't one there, I think your best bet is the Anime Cafe that is hosted by Salt Lake City's Anime Convention, "Anime Banzai".
