The categories that are open only for supervisors are the catch-all categories. Note: 'Catch-all Questions' the category, not the catch-all categories (e.x. 'Adds No Value') is able to be viewed by regular contributors and regular contributors have the ability to add questions there.
No, not in the least. All WikiAnswers Supervisors have the same influence in every category. Category Supervisors usually spend more time in their respective categories, but are in no way restricted to where they can contribute.

Chris Whitten, the founder of FAQ Farm, which later became WikiAnswers, was most likely the supervisor of the first few categories on the site. Others who would have been supervisors of the early categories are Crystal, Neila222 and RoyR.
Tricky one to answer. Supervisors don't supervise a set number of contributors - they supervise individual categories. For example - I supervise the Snakes category. Some categories will be more popular than others - and thus attract more contributors.
