I have a 91 taurus, and re-did the entire wiper system in it. The Main relay is , under the hood, on the right side of the wiper motor, "looking torwards the windshield." It is a black box with a clip and a wiring harness. It can be a little difacult to remove the clip at times. Good luck.
The MAIN fuse box is under the hood on the drivers side on the radiator shroud near the battery. Once you open it, the 'legend' is on the inside of the cover. Before you go yanking fuses, take a minute and orient the corner 'legend' to the tray to make sure you have the right fuse.

Wipers normally use a breaker, not a fuse.

There is no button to operate the headlight wipers. The headlights washers and wipers are activated along with the windshield wipers when you use the windshield washer by pulling the wiper switch towards you.
The fuse for the washer pump is the same as the wipers. The fuse is in the main power distribution box.

Should be the same as the wipers--Check to see if you are getting power to the washer pump - if so could be a bad washer pump
