If you mean "noncombatants," many of our presidents have served in combat...and, sadly, innocent people die in wars. So no, McCain wouldn't be the first president who killed noncombatants.
The Navy is a Military Branch. There are always the risk of injury and death. Ships are dangerous places. And in combat, ships and aircraft, as well as shore installations, are targets for the enemy.

The purple heat is solely awarded for military members who are wounded or killed in combat from enemy actions. It could be from a concussion from an explosion our losing multiple limbs. There are some cases where members in support of the fight such as on a naval ship are injured not directly by enemy action, but while conducting operations against the enemy and can also be awarded the people heart. Other awards are given for actions, service, or performance by a military member. The Purple Heart is awarded for their injuries, not for any actions on the awardee's part.
