They can be gifts or self purchased. Mine was received from my parish priest when I was sent to Vietnam. It finally wore thin and I had a casting made from some scrap gold I had. I still wear mine today. Even though it is called the ESC, it is non-denominational Christian. It was, I am told, designed by bthe wife of an Episcopal Bishop during WWI.
The recipients of the Episcopal service cross are individuals who have demonstrated exceptional service and dedication to the Episcopal Church and its ministries. This may include clergy, lay leaders, volunteers, or anyone who has made a significant impact on the church and its mission. The cross is often awarded as a token of appreciation and recognition for their contributions.

The Victoria Cross is awarded to heros of Great Britain (although recipients are of many nationalities). List of recipients linked below
Parish Episcopal School's motto is 'Wisdom, Honor, and Service'.
There have been 13,446 recipients of the Distinguished Service Cross, far too many to list here. See the link below for cross referenced lists. Perhaps you mean the Distinguished Service Cross, or DSC. Lists can be found in Department of Defense General Orders. The Distinguished Service Cross list for Vietnam, along with the Navy Cross and the Air Force Cross, is listed in the back of the book "Stolen Valor" by B.G. Burkett. Or he might have meant the DSO, the Distingushed Service Order, awarded to 870 RAF officers in WW2. Not everyone's American ya know (edit)
No such comprehensive list exists.
I suggest you start at the National Archives - Victoria Cross registers
As far as I know, there is no list for recipients of the Bronze Star Medal from WWII. The reason for this is, unlike higher awards that are awarded strictly for combat actions (Medal of Honor, Distinguished Service Cross, Distinguished Service Medal- which have lists of recipients) the Bronze Star Medal may be awarded also for non-combat reasons. Because of this, the list would be extraordinarily long, particularly for a period of war.

No, but it has swayed more than one Promotion Board. Metal of Honor recipients are awarded 1000$ extra per month in most cases. Distinguished Service Cross recipients are awarded 20% extra pay for their retirement if they served at least 20 years on active duty. Below that metals are only a way to show the service and improve promotion points for an individual.
NO! While the Purple Heart Medal is awarded for wounds received in combat, there is no pension attached to it, as there are some of the higher awards like the Medal of Honor, Distinguished Service Cross or the Soldier's Medal.
